TOPAMAX Offers Flexible Dosing for Optimal Results
Initial monotherapy dosing in adults, children ≥ 10 years old, and adolescents
- Recommended titration
- 25 mg BID in week 1
- Increase by 25 mg BID per week in weeks 2-4
- Increase by 50 mg BID per week in weeks 5 and 6 to 200 mg BID
- Most of the therapeutic effect of topiramate monotherapy was achieved with plasma concentrations of 4 to 10 µg/mL
Adjunctive therapy dosing in adults
TOPAMAX recommended daily dose for adjunctive therapy:
Initiate therapy with a dose of 25 mg to 50 mg of TOPAMAX.
In adults: 200 to 400 mg/day in 2 divided doses for partial-onset seizures; 400 mg/day in 2 divided doses for primary generalized tonic-clonic seizures.
Adjunctive therapy dosing in children and adolescents
- In pediatric patients, ages 2-16 years (partial-onset seizures, primary generalized tonic-clonic seizures, or Lennox-Gastaut syndrome):
- Titration should begin at 25 mg (or less, based on a range of 1 to 3 mg/kg/day) nightly for the first week
- The dosage should be increased at 1- or 2-week intervals by increments of 1 to 3 mg/kg/day (administered in two divided doses), to achieve optimal clinical response
